Please complete, print and sign the Transfer of Ownership C-0050 form.

In addition, we will need updated banking and beneficiary information. Please complete the PAD Authorization Pre Authorized Debit C-0170 form, and include either a void cheque or a Direct Deposit Form provided by your financial institution, and the Beneficiary Designation D-0017 form and email all three signed forms to us at insurance@empire.ca or mail them to:

Please note, if you are transferring your policy to be owned by a corporation, please complete the Corporate Organization Owner Supplement C-0044 form.

All issued life insurance policies, without a seg fund deposit,  have a 10 day free look period in which the policy can be terminated without penalty. If the policy owner submits a request to rescind the contract, the Company will refund the initial premium to the owner. If you need to request adjustments to your coverage or application after submission, please contact your advisor as there may be additional requirements.
